We are excited to announce we will be offering a Sports Physical Day with Saint Alphonsus. This is for anyone that needs a sports physical for next year that would mean any Freshman and Juniors. As well as any sophomore and senior that need a sports physical.
When: Saturday May 4th
Time: 11:30 - 1:30PM
Where: Saint Alphonsus JPR and Sports Medicine Clinic: 6165 W. Emerald St. Boise, ID, 833704
Cost: $25 Proceeds will be donated to the athletic programs at Capitol and Timberline High School for purchase of supplies related to injury prevention and care of their athletes.
Important Notes: Please Dress in Athletic Attire (Shorts, T-shirt, Tank-top)
The history portion of the physical form must be completed and signed by a parent or guardian prior to admission to the examination session. Additional forms (eg. scout/church forms) will not be signed by the doctors.
